فرهاد بالشزر، شهروند ۵۵ساله‌ی کُرد و از پیروان آیین یارسان، اهل روستای سرخک سنجابی از توابع شهرستان اسلام‌آباد غرب (شاباد) و ساکن شهرک پردیس در استان کرمانشاه بود. او به‌عنوان فردی زحمت‌کش از جامعه‌ی کارگری، سرپرستی خانواده‌ای چهارنفره را بر عهده داشت و نان‌آور اصلی خانه، متأهل و پدر دو فرزند، یک دختر و یک پسر بود که با وجود تمامی محدودیت‌ها و فشارهای معیشتی، برای تأمین آینده‌ای بهتر برای فرزندانش تلاش می‌کرد. در شامگاه ۱۸ دی ۱۴۰۴، حوالی ساعت ۲۲، در جریان فراخوان شاهزاده رضا پهلوی در محدوده‌ی بلوار شقایق شهرک پردیس کرمانشاه، به دست جمهوری اسلامی و عواملش کشته شد و بر اثر شلیک مستقیم گلوله‌ی جنگی از سوی نیروهای سرکوب‌گر به ناحیه‌ی سمت راست سر، جان باخت. به گفته‌ی منابع آگاه، پس از کشته‌شدنش، نهادهای امنیتی تحویل پیکر را منوط به اخذ تعهد، سکوت رسانه‌ای خانواده و عدم برگزاری تجمع عمومی کردند. این فشارها موجب شد احراز هویت و انتشار اخبار مربوط به مزار او در رسانه‌های حقوق‌بشری با چند هفته تأخیر صورت گیرد. سرانجام پیکر او تحت تدابیر شدید امنیتی به زادگاه آباواجدادی‌اش در روستای سرخک سنجابی منتقل و به خاک‌سپرده شد. با وجود محاصره‌ی امنیتی راه‌های منتهی به روستا و احضار و بازجویی از اعضای خانواده، جمعی از نزدیکان و هم‌شهریان کُرد او در مراسم تدفین حضور یافتند. آیین‌های سوگواری و چهلم وی با اجرای آیین‌های سنتی یارسان و ذکر و دعاهای محلی برگزار شد. در مراسم چهلم او در اواخر بهمن‌ماه ۱۴۰۴ بر سر مزارش، حاضران و مادران دادخواه با فرستادن درود بر روان پاک او و دیگر کشته‌شدگان انقلاب شیروخورشید، نام او را به عنوان «شهید راه آزادی» گرامی داشتند.
